Part 1

HIPAA Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully.

Federal law (the Health Insurance Portability and Accountability Act, or HIPAA) requires us to protect the privacy of your health information, to give you this notice, and to follow the terms of the notice currently in effect.

Our duties to you

  • Keep your protected health information private and secure.
  • Give you this notice describing our legal duties and privacy practices.
  • Follow the terms of the notice in effect today.
  • Tell you promptly if a breach affects the privacy of your unsecured health information.

Protected health information includes the details we collect when you book, consult, or receive care, such as contact information, medical history, medications, treatment records, and billing details.

How we use and share information without your written permission

  • For treatment: We use your information to plan and provide your care. Your provider reviews your medications and medical history before recommending a treatment, and we may share information with another clinician involved in your care.
  • For payment: We use and share information to bill for services, including sending claims to health plans and processing HSA card payments for qualifying treatments. We keep a card on file for your appointment, and your provider reviews charges with you before treatment.
  • For health care operations: We use information to run and improve our practice, including quality review, staff training, scheduling, and internal audits.
  • Appointment reminders and follow-up: We may contact you by phone or text about appointments, aftercare, and follow-up visits. You can ask us to reach you a particular way.

Uses that require your written authorization

We will not use or share your health information for the following purposes unless you give us written authorization. You may cancel an authorization at any time, in writing, except to the extent we have already relied on it.

  • Marketing: We will not use your information to market products or services to you, except in face-to-face conversations or for small promotional items we provide during a visit.
  • Sale of information: We will not sell your health information.
  • Psychotherapy notes: We will not share separate psychotherapy notes without your authorization, except as the law allows.

Other uses permitted by law

The law allows or requires us to use or share information without your authorization in certain situations, including:

  • When required by federal, state, or local law.
  • For public health activities, such as reporting certain diseases or product problems.
  • To report suspected abuse, neglect, or domestic violence as the law requires.
  • For health oversight activities, such as audits and licensing reviews.
  • In response to a court order, subpoena, or other lawful process.
  • To law enforcement, when the law requires it or in the limited circumstances the law describes.
  • To coroners, medical examiners, or funeral directors, as permitted by law.
  • For research, only with approval under strict privacy rules.
  • To prevent a serious threat to your health or safety, or the health or safety of others.
  • For workers' compensation claims, as permitted by law.
  • To people involved in your care or payment, when you agree or do not object, or when you cannot decide and sharing serves your best interest.

Indiana law may provide additional protections for certain information, such as HIV-related and mental health records. We follow the stricter rule when state law applies.

We will not retaliate against you for filing a complaint.

Part 3

Medical Disclaimer

General information only

The information on this website, including text, graphics, images, and other materials, is provided for general educational purposes. It helps you learn about the treatments offered at Belle Allure Aesthetics and Wellness and prepare for a consultation. It is not a substitute for professional medical advice, diagnosis, or treatment.

Consult your healthcare provider

Please speak with a qualified healthcare provider about any question regarding a medical condition, medication, or treatment, and talk with your provider before acting on anything you read here. Seeking care promptly is always the right step when you have a health concern.

Consultation before treatment

Treatment decisions are made only after an in-person consultation. During that visit, your goals, medical history, skin or hair history, and current medications are reviewed, and your provider explains which options may be appropriate for you. Prescription medications, including GLP-1 weight management medications, are offered only after a medical evaluation and with ongoing guidance from your provider.

Individual results vary

Outcomes depend on your health history, skin or hair condition, current medications, and how your body responds. Descriptions of results, and any photos on this site, are illustrative and do not guarantee a specific outcome for you.

Injectable and regenerative treatments

Injectable treatments and regenerative therapies that use your own blood carry risks, including bruising, swelling, redness, tenderness, and, less commonly, infection or other complications. Your provider reviews these risks with you before any treatment begins. Please tell your provider about pregnancy or breastfeeding, allergies, bleeding conditions, and any medications or supplements you take.

Medical emergencies: This website is not for emergencies. If you have difficulty breathing, chest pain, swelling of the face, lips, or throat, vision changes, fainting, or any other urgent concern, call 911 or go to the nearest emergency department right away.

No provider-patient relationship online

Reading this website, submitting an online form, sending a message, or calling the office does not establish a provider-patient relationship. That relationship begins at your first consultation and treatment agreement. Please share personal health information at your consultation or through the secure patient portal, rather than through website forms or unsecured messages.

Third-party links and accuracy

This website may link to external sites, including social media, booking platforms, and reference materials. Belle Allure does not control those sites and does not endorse their content. We review our content regularly, and medical knowledge changes over time. We make reasonable efforts to keep information accurate and current, but we do not guarantee that all content is complete or up to date at any given time.

Part 4

Terms of Use

These terms explain how to use our website, how consultations and appointments work, and what you can expect from us. By using this website or booking an appointment, you accept the terms below. We share additional policies at booking and at your visit, and those policies become part of your agreement with us.

Eligibility

You must be 18 or older to use online booking or book a treatment. Minors may be seen only with written consent from a parent or legal guardian.

Care guided by a licensed physician

Belle Allure Aesthetics and Wellness operates in Elkhart, Indiana, under the medical supervision of a licensed physician. Katherine Clark, RN, provides treatments under that physician oversight. Every treatment begins with a consultation, where your provider reviews your health history, current medications, and goals, explains the benefits, expected sensations, and possible side effects, and answers your questions before anything is scheduled.

  • You decide whether to move forward, and you can pause or decline a treatment at any time.
  • Aftercare instructions and follow-up check-ins help you get the most from your plan.

Realistic expectations

Results vary from person to person. Your provider shares realistic expected outcomes during your consultation. Some treatments need more than one visit or ongoing maintenance, and your provider will tell you when a follow-up visit is recommended. Before-and-after photos show individual results and are shared only with patient consent.

Booking, card on file, and cancellations

  • Accurate intake: Complete your consent and health history forms, with current medications, past treatments, allergies, and conditions your provider should know before your visit.
  • Card on file: We keep a card on file to hold your appointment. Payment is handled at your visit, and your provider reviews charges with you before treatment.
  • Rescheduling and cancellations: Call 574-735-1528 to reschedule. Cancellation windows, any deposit, and any no-show fee are listed in the policy you accept when you book.
  • HSA cards: HSA cards are accepted for qualifying medical treatments, such as TMJ therapy, headache treatment, and IV therapy when your provider prescribes it for a medical condition.

Pricing

Your provider shares the exact price for your treatment during your consultation, before you decide. Questions about charges are welcome by phone at any time.

Using this website

The content on this website is general information that works alongside a personal consultation with your provider and advice from your own doctor. You may share links to our pages and use our content for personal reference. Reproducing, adapting, or republishing content requires our written permission. Our online booking, reviews, and social media pages are provided by outside services that have their own terms and privacy practices.

Updates to these terms

We update these terms from time to time and post the revised version on this page with a new date. Continuing to use the website after an update means you accept the revised terms.

Governing law

These terms are governed by the laws of the State of Indiana. Any dispute related to this website is subject to the courts located in Elkhart County, Indiana.
